Nelly is not a practice management system. Nelly is a financial operating system that sits on top of practice management systems, and that distinction is the whole thesis.

The German healthcare sector runs on paper: paper intake forms, paper invoices mailed four weeks after treatment, paper consent documents signed at the reception desk. Nelly doesn't ask practices to replace the software their teams have used for 20 years. It attaches to those systems and automates everything those systems don't touch: patient intake, document signing, billing, payments, and factoring. The mechanism: practices keep their existing workflows while Nelly captures the high-value financial layer above them.

That positioning makes Nelly unusually defensible. Displacing a practice management system is a project measured in months and goodwill. Displacing an embedded financial layer that has already automated billing, accelerated cash flow, and eliminated 10 minutes of staff time per patient requires overcoming a switching cost structure most competitors cannot match.

Founding Story

Nelly was founded in Berlin in 2021 by five co-founders: Niklas Radner (CEO), Lukas Eicher (Co-CEO/CPO), Dr. Tobias Heuer (COO), Rasmus Schults (CTO), and Laurids Seibel (VP Customer Success). The core team knew each other from Klarna. Radner spent 12 years building fintech products across n26, Foodora, and Klarna before founding Nelly at 35.

The origin story is not healthcare-first. During the COVID-19 pandemic, the team built a side project to digitize dog tax registrations using bank-transaction-based signature technology, scaled it to 18 cities, and sold it. The experience was instructive and limiting: the team could either become a government agency vendor or find a larger problem.

The pivot arrived at a dentist's office. Radner watched a severely understaffed practice manage patient intake entirely on paper, then received a mailed invoice for €120 four weeks later. The gap between fintech-grade payment infrastructure and the administrative reality of healthcare was obvious. Fintech veterans with deep experience in payment innovation had a direct application: apply the same process discipline that transformed consumer banking to medical practice operations.

That founding logic still governs the product. Every feature Nelly builds must pass a single filter: does it reduce the number of administrative steps for medical staff? The company operates on a "start small, learn fast" model, with product managers conducting continuous customer interviews rather than building in isolation.

Product

Nelly operates across three functional layers.

Financial Operations and Payments. This is the core value proposition and the primary growth driver. Digital Factoring automates claims management and provides practices with immediate liquidity and default risk protection. KV/KZV Factoring extends this to statutory health insurance billing, giving practices instant payouts on public insurance claims. The online payment module allows patients to settle invoices via smartphone with a single click, supporting Apple Pay, Google Pay, credit card, and SEPA direct debit. A recently launched embedded insurance feature, co-developed with Privadent, surfaces tailored dental supplementary insurance recommendations directly within the patient's digital checkout flow.

Administrative Workflow and Documentation. Digital Anamnesis allows patients to complete medical history questionnaires via smartphone before their appointment. Documents are signed electronically with advanced e-signatures and synced automatically into the practice's existing software. Nelly AI adds voice-to-text transcription and automated session summaries, allowing clinicians to document in real time without switching to a keyboard. A Practice Rating module automatically prompts patients to leave Google reviews upon payment completion.

Platform Core and Infrastructure. The Nelly Connector is the integration bridge that synchronizes data between Nelly and legacy practice management systems. Current integrations include Z1.PRO, Charly by solutio, Ivoris, EVIDENT, Dentport, and others, with medatixx listed as coming soon. Practices also receive physical card terminals for on-site payments and electronic health card reading, bundled into the subscription pricing. The entire platform is hosted on GDPR-compliant AWS infrastructure in Frankfurt, with AES-256 encryption at rest and TLS in transit.

The positioning is deliberate. Nelly describes itself as "an add-on, like a little turbo that you put on top." That framing is strategically precise: it removes the change-management obstacle of full system replacement while capturing the surface area of every financial transaction the practice processes.

Market, Competition & Business Performance

Market

The addressable market is structural and large. Germany operates approximately 100,000 ambulatory medical and dental practices. The majority still manage patient intake, billing, and payments through paper-based workflows or legacy systems with no digital payment capability. Healthcare administration in Germany costs an estimated 30% of total practice revenue, a burden concentrated on staff who could otherwise support clinical work.

Nelly targets the highest-friction segment of this market: practices with high new-patient throughput and a significant proportion of self-paying patients. Orthodontists were the initial beachhead, followed by dentists, GPs, dermatologists, gynecologists, and practices in aesthetic medicine. The company intentionally avoids the hospital sector, where intake processes and software landscapes differ materially from ambulatory care.

Geographic expansion is now in motion. Nelly officially entered Italy in September 2025, targeting the same structural dynamics: fragmented, paper-heavy administrative processes in ambulatory care with limited digital payment adoption.

Competition

The competitive set Nelly lists includes E-clinic, Pabau, CarePaths EHR, Jane, and Medistar. These are primarily practice management and EHR platforms. That framing matters: Nelly does not compete with them directly. It integrates with them.

The more relevant competitive tension sits with legacy factoring providers and billing intermediaries who have operated in the German healthcare market for decades. Nelly differentiates against these incumbents on three dimensions: mobile-first patient experience, transparent unified pricing for terminals without complex bank contracts, and a single point of contact for billing, factoring, and hardware issues. Traditional providers require practices to manage separate vendor relationships for each function.

The moat compounds through integration depth. Every practice management system Nelly connects to becomes a distribution node, not a competitor. The proprietary connector technology that ties Nelly's financial layer to legacy software is itself a barrier: replicating it requires building individual integrations with every relevant PMS in the market.

Business Model

Nelly charges practices a monthly SaaS subscription across four tiers.

Starter (from €99/month): Standardised digital anamnesis, document send and receive, direct PMS integration, individual documents, one free card terminal, regular data analysis.

Standard (from €199/month): Individual digital anamnesis, PMS integration, six individual documents, one free card terminal, data analysis. Includes two card terminals.

Premium (from €299/month): Individual digital anamnesis, PMS integration, twelve individual documents, two free card terminals, data analysis.

Enterprise: Custom pricing for clinics and multi-site operators.

The SaaS subscription is the floor, not the ceiling. Digital Factoring and KV/KZV Factoring generate transaction-based revenue as a percentage of processed claims. Card terminals bundle hardware into the subscription. The embedded Privadent insurance product introduces a referral or commission revenue stream. The architecture is land-and-expand: acquire practices on the subscription, then grow revenue per customer as they adopt more financial products.

Traction

Nelly reached 1,200 practices by September 2025, up from 1,000 at some earlier baseline, with more than 2 million patients digitally processed through the platform. The company has digitized over 30 million paper documents. The team grew from four co-founders to 85 employees within three years. More than 10% of company shares are reserved for an ESOP.

In January 2025, Nelly closed a €50 million Series B led by Cathay Innovation, with participation from Notion Capital, b2venture, and Lakestar. The round funds product line expansion and geographic growth. In November 2025, CEO Niklas Radner appeared on the Financial Times' "The Next Five" podcast to discuss European tech investment and the company's funding trajectory.

The claimed efficiency metrics, 80% reduction in paper costs, 10 minutes saved per patient, 10% more treatments performed, and 30% revenue increase, are practice-level outcomes that function as the sales thesis. Whether they hold at scale across a diverse customer base is the key question for the company's next growth phase.

That question is also the growth opportunity. If Nelly can demonstrate those outcomes consistently across 5,000 practices rather than 1,200, the compounding effect on revenue per customer and net retention makes this a very different business.
